Insurance Coverage

Clarity Rose is in-network with: United Healthcare, Carelon (Maryland Medicaid), Carefirst (Blue Cross and Blue Shield), Carefirst FEP, Aetna, and Wellpoint (DC Medicaid). For dietitian services, Clarity Rose is in-network with Aetna and United Healthcare, but in the credentialing process for Carefirst. Dietitian services are not in-network for DC Medicaid or MD Medicaid.

Clarity Rose keeps 3 spots for Medicaid patients. Ask to be placed on the Medicaid waitlist.

If Clarity Rose does not accept your insurance carrier, patients can pay the out-of-pocket fee-for-service and receive an itemized bill (i.e., superbills) to submit for reimbursement to your insurance carrier. See below for out-of-pocket rates.

Payment Policy

Session payments are required for services to continue. Services can be discontinued at any time, at the discretion of the provider, if session fees are not paid or if a large balance has accrued. Sessions will not occur, unless the patient is in a crisis situation, if payment cannot be made.

Missed appointments without notice or appointments canceled without 24 hour notice will result in the patient being charged the full fee-for-service cost of the session. This policy is also applicable to scheduled assessment appointments. Any outstanding balances are subject to be sent to collections, if the balances are not paid towards each week or paid in full upon request.

This fee schedule is for self-pay patients who are either out-of-network for the Institute or plan to submit payments to their insurance company at a later date utilizing a superbill.

Therapy Fee Schedule

Therapy services start at once per week minimum, but can be twice per week if patient’s symptoms indicate increased need. Sessions will not start bi-weekly, but can decrease to bi-weekly or monthly, as patient symptoms indicate. All bi-weekly or monthly patients will be see on Tuesdays.

Individual Therapy

Dietetic and Nutrition Fee Schedule

Appointment frequency will vary based on patient need. For patients with moderate to severe eating disorder or weight management concerns, a commitment to weekly sessions may be necessary.
